Single Point of Truth: Why your AI hallucinates without one

Feed your AI every document from the last ten years and you build a hallucination machine. The fix is unglamorous: one single, living master document per topic.

Felix Degeler, black-and-white portrait during a talk.

An AI doesn't know what's current. It doesn't know that a decision from 2018 was overruled by an email last week. Feed it everything that has piled up over the years and you won't get reliable answers, just random results that sound convincing and are wrong anyway.

The principle: one document, one truth

The approach is radically simple: every important topic gets exactly one document that is the single, current truth, the Single Point of Truth. Everything else gets archived. The AI only ever accesses this one living master document.

That keeps the system clear and reliable. Instead of guessing between five contradictory versions, the AI has one source that is maintained and always correct.

Why this holds without AI too

The principle is older than any chatbot. Projects rarely fail from incompetence, usually from chaos: calendar here, notes there, project status somewhere else. One central place with everything (plan, to-dos, decisions, results) creates clarity and makes energy flow into the solution, not the search.

What this means in practice

Before rolling out AI in your company, answer one question: where does the truth on this topic live, and is it current? As long as that's open, AI just accelerates the chaos instead of solving it.
